What is SAGE? The SAGE Initiative: Stability, Advocacy, Growth, & Empowerment is a program intended to address the ongoing needs of members of the adoption constellation beyond the limitations of an open child welfare case. We can provide specialized case management, collaborative problem solving, and connection to long term support services.
What services does this program provide? We will provide training, expertise, short-term case management, family counseling, resource gathering, and connection to long-term supports. Our goal is to meet every individual and family where they are at and collaborate to create a plan that addresses their needs.
Who does this program serve/who is a good fit for this program? SAGE will serve any member(s) of the adoption constellation: birth parents, children/youth/adults impacted by the child welfare system, adoptive parents, kinship providers, and more
